My Outlook has 4 accounts set up. One is a work account and uses a signature, the other 3 are personal accounts and only one of those accounts has a signature. Not sure if safe mode will prove anything as rebooting Outlook fixes the issue for a short time. Not sure if this helps, but I forgot another thing that it does with regard to signatures.
